• Passport
• Scanning of the main page of the passport details
• Scanning of the page of the passport cover
• Scanning of KITAS Stamp on the passport
• Copy of Original KITAS with legalize Stamp by Immigration office that issued KITAS
• Marriage / Divorce Certificate (in English / Indonesia) - if any
• Birth Certificate (in English /Indonesia)
• Formal Photo (with red background)
• Address in (must be same with KITAS address)
• Valid Domicile Letter from Neighborhood Head
• Domicile Letter Approval by subdistrict (if any)
• KTP (ID Card) & Kartu Keluarga (Family Certificate) Sponsor
